OriX4D Orofacial Dynamics (“OriX4D”) and OraShape are not a provider of medical, dental or health care services and does not and cannot practice medicine, dentistry or give medical advice. OriX4D or OraShape will not initiate, edit or modify a patient’s True Setup treatment plan. OriX4D or OraShape may from time to time provide information or recommendations to provider; however, OriX4D or OraShape will not control or pressure provider in the exercise of provider’s professional judgment.
Decisions about treatment of a patient are to be made by provider, and provider is solely responsible fo review and approval of True Setup treatment plans to determine whether OraShape products or treatment options are appropriate for a specific patient, use or application, diagnosis and treatment of each patient and the resulting outcome. Some OriX4D product or treatment option offerings incorporate default or preset treatment staging protocols which are accepted by the provider as part of his/her prescription when submitting an order to which these protocols apply. Information about treatment staging protocols can be accessed via the OraShape Portal. It is within provider’s sole discretion to implement any of OriX4D’s or OraShape’s recommendations, including but not limited to proceeding with order submission.
Each order is subject to acceptance by OriX4D and OraShape, and will become a binding contract only if accepted by OriX4D and OraShape. OriX4D treatment options are only available to currently licensed orthodontists. OriX4D treatment options are only available to orthodontists and providers that have maintained their customer standing. OriX4D or OraShape may require provider to submit feedback regarding a patient’s treatment with the OraShape System upon case completion.
True Setup software is a computerised approximation of desired tooth movement for a specific patient.True Setup software results are simulated approximations based on the approved True Setup treatment plan; therefore, actual clinical results may vary. Provider is requested to review the True Setup treatment plans thoroughly. Many providers make changes prior to approving the final True Setup treatment plan.Provider’s approval of the customised True Setup treatment plan is considered a prescription to OriX4D and OraShape’s final authorisation to manufacture the aligners. Prior to use of the True Setup software, provider agrees to review and approve the terms of the True Setup Software License Agreement.OriX4D recommends that provider delays extraction of any teeth until after approval of the True Setup treatment plan, if medically feasible.
